Insurance Information

MASTER GLOBAL BLANKET ACCIDENT AND SICKNESS INSURANCE PROGRAM

PERTAINING TO:
PARTICIPANTS ON UNITED JEWISH COMMUNITIES, INC. (UJC)
SPONSORED MISSIONS WHILE OUTSIDE THEIR COUNTRY OF RESIDENCE
including all Full-time employees of UJC and the Federations of North America

POLICY DATES:   2/20/2008 – 2/20/2009



SCOPE OF COVERAGE:
This is a comprehensive Travel Insurance Program which insures Mission participants, including staff, on a world-wide basis.  Benefits are provided for Accidental Death or Dismemberment, Excess* Medical and Hospital Expenses, and other Travel related losses as set forth in the attached Schedule, including accidents resulting from Terrorism or Acts of War.

KEY EXCLUSIONS:
Claims Arising from:
•    alcoholism and drug addiction;
•    intentionally self-inflicted injury;
•    piloting or serving as a crew member or riding in any aircraft except as a fare-paying passenger on a regularly scheduled airline;

Medical Expense Benefits Exclusions:
•    routine physicals;             
•    routine dental care;  
•    cosmetic surgery except for reconstructive surgery following an injury;
•    mental and nervous disorders;
•    pregnancy or childbirth, unless treatment is required in a medical emergency;
•    routine nursery care;
•    eye refractions;
•    services not deemed medically necessary and reasonable by a doctor
•    treatment provided by private duty nurse;
•    treatment by any immediate family member of member of the household;

*The Medical and Hospital Expense Benefit s are paid when the Covered Expenses are in excess of the amounts paid by any other Health Care Plan, as well as a $25 Deductible.   In cases where there is no other Insurance Coverage, or where the other Health Care Plan denies Coverage, when this Policy would otherwise provide Coverage, this ACE Program will pay such expenses, subject to a $25 Deductible.

PERTAINING TO:

INDIVIDUALS TRAVELING TO ISRAEL ON UNITED JEWISH COMMUNITIES, INC. (UJC)
AND/OR
UNITED ISRAEL APPEAL OF CANADA, INC. (UIA FEDERATIONS OF CANADA)
SPONSORED MISSIONS

ARARAT (CLAL INSURANCE COMPANY, LTD.)         See attached SCHEDULE          
POLICY DATES:    2/1/2008 – 1/31/2009                      

SCOPE OF COVERAGE:
This Program, which insures Mission participants, applies only for the duration of the mission and only in Israel, including the occupied areas, the Autonomy, Jordan and Egypt.  Benefits are paid for Accidental Death or Dismemberment, Medical and Hospital Expenses, and for other Travel related losses as set forth in the Schedule attached.

Benefits are paid for losses resulting from Terrorist Acts as long as the individual insured did not take part in the acts and/or was not on Active Army Service at the time.  Benefits will also be paid where loss is due to Passive War Risk*, defined to mean actions taken by enemy or hostile forces where the individual insured did not take part in the acts and/or was not on Active Army Service at the time. 

*Except in the Occupied Territories and the Autonomy Areas A & B (where Israel does not hold both Civil and Security control).

KEY EXCLUSIONS:
Claims arising from:
•    Engaging in a Sports Activity
•    War (other than Passive War)
•    Radioactive, Biological or Chemical Weapons
•    Self-Imposed Injury or Illness
•    Use of Alcohol or Drugs
•    Pre-Existing Medical Conditions
•    Medical Expenses due to Malignant or Chronic Illness, Pregnancy, Aids and Organ Transplants

Excluded "dangerous" Activities include:
•    Flying in any Army Airplane or Helicopter
•    Cruising in any Army Sea Vessel
•    Participation in any Fire Shooting (Mitvach), or any usage of Guns
•    Diving when Diving Equipment Required
•    Water Sports other than Rafting and Kayaking
•    Winter Skiing
•    Entering Caves on Ropes
•    Bunjee Jumping
•    Air Gliding
•    Aircraft or Boat Usage except where operated by Approved Companies and Approved Team
